Mr. Smith is a 75-year-old male admitted to your unit with a diagnosed urinary tract infection. Mr. Smith is alert but displaying signs of confusion. Vital signs are ordered to be assessed every 4 hours. Mr. Smith is experiencing urinary frequency, and needs to be reminded to ring his bell or use the urinal rather than getting out of bed on his own to use the bathroom.
